The purpose of PACC is to promote the perspective among CHWs in health settings that regular physical activity plays as important a role as prescription medication in the prevention and management of chronic disease.

The objective of PACC is to train CHWs in the presentation and implementation of therapeutic fitness strategies which emphasize self-care and promotion of healthy lifestyle behaviors to reduce chronic disease and its accompanying comorbidities impacting communities with the highest health disparities.
Physical Activity (PA) is an evidence based prescription for chronic disease.
Peer learning through group discussion, teach back, and hands on physical fitness activities (low-moderate intensity)
Therapeutic fitness modalities must be at the center of patient care.
Develop strategic action plans to improve the delivery and outcomes of PA healthcare services
